Notice to Employees of Allendale County Hospital
ACH is not required to send proof of health insurance (1095C) to employees unless the employee requests at copy.
Employees may request a copy of your 1095C by sending an email to Tammy Smith (
tammys@achospital.org) or by stopping by the Administration Office and notifying Kristie Hutson that you would like a copy of your 1095C.

All requests will be fulfilled by the later of January 31, 2026 or within 30 days of an employee’s request.
